Initial information required:
1. Original Policy and completed Claim Form;
2. Copy of your ID and proof of address;
3. The original death certificate that will be certified as a true copy by our branch office staff. The original document will be immediately returned.
Upon receiving a claim submission, we will advise if any additional information is required.
